Online Financial Loan Application


Before You Submit

You will need a digital copy of the following supporting documents prior to submitting your online application.

  1. Attach a formal, written quote on a vendor’s company letterhead, including ordering information, the requested device(s), total cost of the device(s) - including taxes and shipping. If this is for a vehicle, the quote should include all details, such as the VIN, mileage, and a listing of associated costs that will be rolled into the loan, such as title, registration, etc.

  2. Proof of income.

    • If self-employed, attach your IRS tax returns (including all schedules) for the past two years.

    • If employed, attach at least the last two (2) months of pay stubs from your employer(s) and the last two years of W-2’s from all employers.

    • If you receive SSDI, SSI, VA or other government benefit payment, attach your most recent award letter that you received showing your benefits income.

      *If you have additional income such as retirement, pension, property rental, child support, etc., acceptable documentation includes:

      • a) copies of court documents verifying this income

      • b) copies of IRS tax returns (including all schedules) for the past two years

      • c) copies of your most recent retirement or pension statements

      *** Please note that bank statements showing income deposits are no longer accepted by our lending partner for the Financial Loan program.

  3. Identification (must not be expired). A copy of one of the following.

    • US Driver’s License (any state)

    • US Passport (must include signature portion)

    • US Armed Forces Identification

    • Identification issued by a state agency

  4. Proof of Massachusetts residency (must not be expired). A copy of one of the following.

    • Massachusetts Driver’s license (satisfies both MA Residency and Identification criteria)

    • Voter’s registration

    • Utility bill in your name

    • Non-driver’s, government-issued identification

  5. Acceptable documentation of disability. You only need a copy of one of the following.

    • A signed doctor’s letter

    • SSDI or benefits award letter

  6. If applying for a modified vehicle loan:

    • Massachusetts Driver’s License

    • Any documents listing a lien holder (such as the vendor quote), must list: Berkshire Bank, PO Box 186, East Syracuse, NY 13057

    • An insurance binder/proof of insurance will be required prior to payout with the loss payee listed as: Berkshire Bank, PO Box 186, East Syracuse, NY 13057

In addition to including your supporting documents, please make sure that you also…

  1. Complete all required sections of the application.

  2. Sign and date the Certification, Authorization, and Information Release forms.
